Application Procedures

All Foundation requests must be made in writing. Proposal seekers are advised to submit a 3-5 page proposal including detailed budget (all pages must be numbered). A separate cover letter summarizing the proposal request is also required. Please do not send videos, DVDs or CDs.

In order to be considered for funding, proposals must meet the following procedures. Those that do not meet these requirements will not be considered.
  • Proposal Summary Letter (do not staple proposals)
  • 3-5 Page Proposal including:
    • Organization Background - Briefly describe history, goals, objectives, and accomplishments
    • Project Description - Clearly describe project activities, goals, objectives, and project time frame. Also include qualifications of staff involved in implementing project
    • Results - Define specific quantifiable outcomes projected as a result of the program or project
    • Budget Allocation - Include a description of the total project budget with a detailed breakdown itemizing monies requested from the Foundation
    • Post Funding - Indicate how program activities are to be sustained after funding from the Foundation
    • Program Dissemination - Outline program plans for dissemination and replication
  • Board Listing - Current list of members and their affiliations
  • Financial Statement - Copy of most recent audited statement
  • Operating Budget - Copy of organization's current general operating budget
  • Investors - List of current public and private investors, including their levels of support
  • 501(c)(3) Document
